Case Studies on Two Cases of Adolescent Psychiatric Inpatients with Glue Sniffing of in Korea




Abstract
Summary of the Results and Conclusions:
Glue sniffing, the deliberate inhalation of vaporized plastic cements, is a practice of increasing frequency among adolescents. Sensations resembling acute alcoholic intoxication are induced by self-exposure to the volatile organic solvents variously present in plastic glues.
Symptoms vary from mild euphoria and exhilaration and coma after prolonged inhalation. Physiological adci¢¥ction evidently does not occur, but psychological dependency is common. And cement fumes are potentially injurious to liver, kidneys, brain and bone marrow.
The authors have seen two cases of adolescent glue sniffers, admitted to Department of Neurology w Psychiatry, Churg Aug University Medical Center, Seoul, Korea on june 1¢¥x`!1. The summary of the results are described as follows:
1) Both the first case with one month history of glue inhalation and more unfavorable prognosis and the second case with five months history of glue inhalation and more favorable prognosis are 18-year-old school boys, the last son in their sibling order and came from the middle class of their socio-economical status.
2) In the patterns of the glue sniffing by the use of plastic cements, they use more f;-e-luently(3 times in a day), and take larger amount of glue (100 tubes and more) during a shorter period of months. They used to inhalate glues with their peer groups, and they neglect a great deal of concern and warning, expressed by their parents, although they fully aware of halmful effect of the glues sniffing. Their two kinds of motives are the power seeking and adventure seeking and their major symptoms vary from mild euphoria and exhilaration to aggressive acting out and hallucinatory experience after prolonged inhalation Of glue sniffing.
3) Although all cases are diagnosed as substance use disorders, the first cases with poor prognosis have some psychopathic problems such as poor impulse control, lower frustration tolerance, aggressive acting oat, and other antisocial behaviors. But the second case with good prognosis shows emotional troubles such as depression, anxiety, fear, longing for affectional security, and treated by short-term hospitalization and supportive psychotherapy.
4) As to the characteristics of the behaviors and attitudes of the glue sniffers, we found that they have athletic physique, ugly facial appearrance, unfavorable nicknames, future expectation to search for masculine sports and athletic jobs, and school norm deviated behaviors such as poor school performance, truancy from school, and more deviant attitude toward their teachers. They stayed away from home a good deal(4 times or more), smoked a pack of cigarrette daily, drink alcohol once in a week regularly, and commited self-injury and saicidal attempts. They also have problematic father such as alcoholic father, indifferent father and rejecting father in their child rearing practices.
CONCLUSIONS
Through comprehensive analysis of the family history, developmental history, substance use history, hospital history, and psychiatric & physical findings, we found that all cases of t glue sniffers are classified as two types; the psychopathic type and the affective type.
-rang them, the affective type shows more favorable prognosis than the psychopathic glue s:nffers. This findings suggested that in-depth psychiatric evaluation of their personality structures in glue sniffers must be needed to set up treatment and guidance plan.
In order to protect the perplexed adolescent glue sniffers from harmful effects of the glue sniffing, the comprehensive educational and psychiatric guidance program must be settle down. And their parents and teachers must be on the alert for the behavior changes of their children such as changes of school performance, neglecting home work, tardiness or truancy from school, runaway from home, late night outing, and mingled with bad companions, etc.
